I would start by searching your log files for clues as to why this message was accepted.

Are you using spamfilter to send email as well as receive it?

Log files show: RCPT TO: name@domain.com accepted
Why this? When I only have one domain in my Local Domain-list
(and that's not the same domain as on RCPT TO accepted)
And Whitelist Authorized TO is empty.
 
I send mail trough my mailserver.

Hi !
I found the problem(took a look in the mirror)!
 
When I re-installed the program all over again and again...,
I was in hurry because I was immediately Spammed from Korea...
So I ended up messing with one response code!
 
It seems that if you have wrong in your response-code(s),
SpamfilterISP becomes Open Relay.
 
Now it seems to work ok again...and quite effectively too!
