We use mailmax for our hosting server.  I have several clients on dynamic dsl and cable links whose isp is port blocking.  We set up an additional port to bypass the isp's port blocking. 

I set up rules to allow my local private network to be able to send email without authenication, while all public addresses must use smtp authenication.

My spamfilter forwards email to mailmax on our private network so the mailserver never has to authenicate. We use a simular setup for web forms whick also connect to the mail server through the private network.

I looked into the content filtering on the ability mail server we are currently running, and I found a setting to allow or block a sender based on if they exist in the system.  Along with whitelisting a couple of IP's, this should accomplish what I am trying to do.

We had the same problem using Imail server and switched to Merak. You need to find a mail server that has a good content filtering system that allows you to set a rule to only accept mail for delivery if the sender is an authenticated user or the IP address is on your local IP (or acceptable IP) list.

We have to keep our physical mail servers port 25 open for our users to authenticate and to send outgoing mail. We are not an ISP but a webhosting company and can't use strictly IP rules to prohibit sending. Works great for us.

The problem I have at the moment is instead of all incoming mail going through the Spam Filter ISP program, some spam is being sent straight to the mail server, even though the MX record only points to the IP Spam Filter ISP listens on.

I am trying to find a mail server that will only accept incoming mail from one static IP address, but allow authenticated users with varying IP addresses to connect and send mail.
